In order to be compensated by Glynwood as a contractor, vendor or program participant seeking reimbursement, you must fill out, sign and submit our organization's Vendor Code of Conduct and a W-9 form. Waiver and Photo Release

If you are participating in a Glynwood program such as a training or workshop, please fill out this photo release form and waiver. If you are a contractor doing other forms of work and will not be documented, you do NOT need to fill out this form.
